How to Replace a Lost Medicare Card
Medicare is a program funded by US government which provides affordable health care to citizens above the age of 65. A red, white and blue Medicare card wiil be given to citizens as a proof . Whenever you are seeking healthcare under medicare program, production of medicare card is a must.If your card has been destroyed, lost or stolen, you need to get a replacement card as early as possible. Here I will describe the process of getting a replacement medicare card.
The request for a new medicare card can be made online and the application process is quite simple. Go to the the ocial ecurity administration website and click on the medicare card section. Choose the tab "Replace your lost stolen or damaged Medicare card". You will be asked to fill an online application form here. Make Sure you have the following details handy before filling the form.
1. Details of the lat medical bill which was paid under medicare program.
2. Your Social Security Number and Date of Birth.
3. Your address of communication
You may request a replacement medicare card through phone also. Call Social Security toll-free number at 1-800-772-1213 and register your request for a new card.You will be asked to give the above mentioned details here as well. You can request a replacement Medicare card in person by visiting your local Social Security office.
Once requested, it will take approximately 30 days to get a replacement medicare card mailed to you. In emergency situations, You may require a medicare card within this period. You may get a temporary letter from your local Social Security office to prove that you are a beneficiary of medicare program. This letter can also take upto one week to reach to youonce you make a request.
